Step 1: To begin, access Google Drive HERE. Sign in with your Gmail. If unfamiliar with creating, register for Gmail, and set up Google account to access this feature.

Step 2: Select New -> More -> Google -> Forms. Below, choose Blank Form to start from scratch.

Step 3: Depending on the functions you need, customize your form to your liking.
1. Form Title: where you name your form
2. Form Description: jot down specific notes for your form
3. Questions: this is where you pose your inquiries
4. Options: this section comprises answers if it's a quiz or a text segment recording the respondent's information
5. Answer Format: depending on your preferences
6. Duplicate Question: use to create questions of the same type as the selected one
7. Delete Question: removes the selected question
8. Required: choose this if you want the information to be mandatory (if respondents leave it blank, the form won't be completed and sent)
9. Add Another Question: this section will generate a question with the answer you choose (different from section 6 in terms of selecting answer type)
Step 4: Various types of answer formats
1. Short Answer (1 line)
2. Long Answer (multiple lines)
3. Quiz: Multiple choice, you create preset answers for participants to choose from
4. Checkbox: Checkboxes, participants will tick or leave blank the boxes you set up
5. Dropdown: you provide answers in a list, participants will select one from within
6. File Upload: Questions allow respondents to submit existing files to answer. Note: if you want answers in a specific file type, select Only allow specific file types. You choose the maximum number of files (from 1 - 10) and the maximum file size (from 1MB to 10GB).
7. Linear Scale: Similar to multiple choice but the answers will be fixed as natural numbers increasing, usually used in assessments from low to high
8. Grid Quiz: Used to set up multiple-choice answers and only one answer is selected. Choose Add Other if the answers you provide are not options for respondents.
9. Grid Checkbox: Used to set up multiple-choice answers and can choose more than 1 answer.
10. Date: This section is very suitable for History questions.
11. Time: Used to supplement time.

In the settings section: This is also quite important, so pay attention. To set up, you click the gear icon as shown in the picture below:

- In the Quiz tab within settings, you enable the Set as Quiz option then can consider some options for respondents as below. Next, you press Save to apply the settings.

Step 5: In the SEND section, you can choose to send the form to an individual or individuals via email, you can also choose the share link option to survey a large group.

And results will be displayed in the form of Charts and will be continuously updated.
If you want to view as Excel for easy printing, sharing, and downloading results to your device, then click the Create spreadsheet icon.
You will see information about responses displayed in Google Spreadsheet (new tab). To download the Excel file to your computer -> click File -> Download as -> choose the format you want to download.
To stop receiving responses for the form -> In the Responses tab -> you turn off Accept responses and you're done.
