2. Date, month, and last 2 digits of the year of birth: Write exactly as stated on the birth certificate.

5. Permanent residence
- Write the province code (city code), district code. Only write the commune (ward) code for communes (wards) classified as particularly difficult by the Ministry of Education and Training.

- Candidates eligible for priority based on their permanent residence must confirm having a permanent residence in area 1 for over 18 months or in particularly difficult communes (wards) for over 18 months during their high school years by marking the corresponding box.
6. High school or equivalent educational institution
- Grades 10, 11, 12: Write the name of the school, district (quận) address, province (thành phố) address on the dotted line.
- In the first 2 boxes, write the province code where the school is located, in the next 3 boxes, write the school code. According to the Department of Education and Training, if the school code has 1 digit, write 0 in the first box, if the school code has 2 digits, write 0 in the first box.
- Grade 12 class: Clearly write the class name the candidate is enrolled in, such as 12B9, for independent candidates write TDO.
7. Phone, email: Candidates should provide their phone number and email if available.

11. Independent candidates
- For independent candidates:

12: Exam location
Candidates registering to take the exam at which exam cluster should write the name and code of the exam cluster specified by the Ministry of Education and Training in the corresponding position, similar to the first part of the exam registration form.
13. Exam Registration Location
Students in grade 12 of high school at which school should submit their exam registration form at that school. Other individuals should submit their exam registration form at locations specified by the Department of Education and Training.

14. Exam/Subject Registration
- For current grade 12 students (not yet graduated from high school), they must register for the exam at point a and are not allowed to choose the components of the exam at point b. Specifically:
Choose 3 independent exams: Mathematics, Literature, Foreign Language, and 1 of the 2 combined exams in Natural Sciences (Physics, Chemistry, Biology) or Social Sciences (History, Geography, Civic Education) depending on the chosen combination of subjects for university admission.

15. Exemption from Foreign Language Exam
