The registration and admission form will consist of 4 sections:
Department of Education and Training ... Department Code ...: For registration, candidates should write the name and code of the department they are registering for (2 digits). For example, if a candidate is registering for the Nam Dinh Department of Education and Training with code 25, the candidate should fill in Nam Dinh and 25.

Ballot number: This section is filled by the registration center, so candidates do not need to fill in this section.
A. Personal Information
1. Full name of the candidate: Candidates should write their full name exactly as stated on their birth certificate, ensuring that it is written in capital letters with accents.
Gender: If the candidate is female, write 1, otherwise, write 0.
2. Date, month, and last 2 digits of the year of birth: Write exactly as stated on the birth certificate.
If the date or month is less than 10 (from 0 to 9), the candidate needs to write a 0 before the number. For example, if the candidate was born in February, write 02 in the month box.
3. Place of birth: Candidates only need to write the name of the city or province where they were born. If the candidate was born abroad, write the name of the country using Vietnamese phonetics.
4. Ethnicity: Write according to the birth certificate.
5. Identification Card/ Citizen Identification Card Number
- If the candidate uses the old identification card (with 9 digits), leave the first 3 boxes blank and write the last 9 digits in the rightmost 9 boxes.
- If the candidate uses the new identification card/citizen identification card with 12 digits, write each digit in each box.
5. Permanent residence
- Write the province code (city code), district code. Only write the commune (ward) code for communes (wards) classified as particularly difficult by the Ministry of Education and Training.
- After filling in the province, district, commune codes in the blank box, candidates should clearly write the province (city), district (quận), commune (ward) names on the line below.
- Candidates eligible for priority based on their permanent residence must confirm having a permanent residence in area 1 for over 18 months or in particularly difficult communes (wards) for over 18 months during their high school years by marking the corresponding box.
6. High school or equivalent educational institution
- Grades 10, 11, 12: Write the name of the school, district (quận) address, province (thành phố) address on the dotted line.
- In the first 2 boxes, write the province code where the school is located, in the next 3 boxes, write the school code. According to the Department of Education and Training, if the school code has 1 digit, write 0 in the first box, if the school code has 2 digits, write 0 in the first box.
- Grade 12 class: Clearly write the class name the candidate is enrolled in, such as 12B9, for independent candidates write TDO.
7. Phone, email: Candidates should provide their phone number and email if available.
For candidates requesting online admission registration adjustments, it is necessary to register their personal mobile phone number to receive a one-time password (OTP) via text message for security purposes.
8. Contact address: Candidates need to clearly state the contact person's name, specific address including house number, neighborhood, commune (ward), district (quận), province (thành phố). This address is used to receive the admission notification when candidates are accepted.
B. Examination registration information
9. Candidates using results for university, college, vocational college admissions
- Mark (X) in the box if the candidate wishes to use the exam results for admissions to universities, colleges, vocational colleges in the teacher training field.
- Leave blank if not applicable (the results will not be used for university or college admissions).
10. Candidates following the high school or vocational education program
- If the candidate follows the high school program, mark X in the box for candidates following the high school program.
- If the candidate follows the vocational education program, mark X in the box for candidates following the vocational education program.
11. Independent candidates
- For independent candidates:
+ If not yet graduated, mark X for independent candidates who have not yet graduated from high school.
+ If already graduated, mark X for independent candidates who have graduated from high school.
12: Exam location
Candidates registering to take the exam at which exam cluster should write the name and code of the exam cluster specified by the Ministry of Education and Training in the corresponding position, similar to the first part of the exam registration form.
13. Exam Registration Location
Students in grade 12 of high school at which school should submit their exam registration form at that school. Other individuals should submit their exam registration form at locations specified by the Department of Education and Training.
The registration unit code should be written according to the instructions of the registration receiving location.
14. Exam/Subject Registration
- For current grade 12 students (not yet graduated from high school), they must register for the exam at point a and are not allowed to choose the components of the exam at point b. Specifically:
Choose 3 independent exams: Mathematics, Literature, Foreign Language, and 1 of the 2 combined exams in Natural Sciences (Physics, Chemistry, Biology) or Social Sciences (History, Geography, Civic Education) depending on the chosen combination of subjects for university admission.
- For independent candidates, depending on the purpose of the exam, and the chosen combination of subjects for university or college admission, they may choose either the entire exam (at point a) or select only some component subjects (at point b) as appropriate.
- Candidates register for specific exam/subject components by marking (X) in the corresponding exam/subject component box. For the Foreign Language exam, candidates fill in the corresponding language code: N1 - English; N2 - Russian; N3 - French; N4 - Chinese; N5 - German; N6 - Japanese.
C. Information for High School Graduation Recognition
15. Exemption from Foreign Language Exam
For candidates seeking exemption from the Foreign Language exam, it is necessary to specify the type of certificate qualifying for exemption or indicate membership in the national team participating in international language Olympiads.
16. Registration for Exam/Subject Reserve
Candidates who took the high school graduation exam in the previous year and have subjects eligible for reserve as per regulations may choose which subjects to reserve by recording the corresponding subject scores.
For reserved subjects, candidates can still register for the exam (either the whole exam or subject components) in Section 13 if they wish to use the exam results for university and college admissions.
D. Information for University, College, and Vocational School Admissions
17. Priority Admission Categories
Candidates self-identify priority admission categories and accurately record the symbols for priority categories as prescribed.
Failure to truthfully declare will be handled according to current regulations. Candidates in priority categories must submit all required legal documents to the school upon enrollment.
18. Admission Areas
For candidates aiming to apply for university or college admission, refer to the 2019 priority admission area list to correctly input the area code in the blank box as follows:
- Area 1 (A1) input the number 1;
- Area 2 rural (A2-R) input 2R;
- Area 2 (A2) input the number 2;
- Area 3 (A3) input the number 3.
During the 3 years of high school or equivalent, candidates residing longer in an area are given priority in that area.
19. Year of High School Graduation or Equivalent
Candidates who have graduated should write their graduation year, candidates currently in 12th grade and those who have not graduated should write the graduation year as 2019.
20. For candidates taking the exam for lateral admission to colleges and universities
For candidates taking the exam with the purpose of obtaining results for lateral admission to colleges and universities, mark (X) in the box corresponding to the level completed: Completed intermediate level (IL) or Completed college level (CL).
21. Candidates registering preferences for admission to universities, colleges, and vocational schools
This section is for candidates wishing to apply for admission to universities; colleges, vocational schools in the teacher training field.
To fill in the information in this section, candidates need to thoroughly understand the admission information of the desired school posted on the Ministry of Education and Training's Admission Information Portal and the information page of the schools to have accurate registration information about school code, major/teacher training field code, major/teacher training field name, subject combination for admission.
