First and foremost, the crucial step for a European trip is filling out the visa form. Though the form is just a couple of pages, it can sometimes be a bit tricky. Below, I'll guide everyone on how to fill out the Schengen visa form (European visa).
1. Surname (Family name): The applicant's last name
2. Surname at birth: The applicant's last name before any changes. In Western countries, the bride typically adopts the groom's last name after marriage. Here, you fill in your current last name.
3. First Name(s): Applicant's Given Name(s)
4. Date of Birth: Specify the birthdate in the order day – month – year
5. Place of Birth: Indicate the birthplace exactly as stated in the passport.
6. Country of Birth: Hometown as per passport details (Vietnam)
7. Current Nationality: Present citizenship – Vietnamese
Nationality at Birth: Citizenship at birth – If you ever held a different citizenship, provide details here.
8. Gender: Male / Female
9. Marital Status: Indicate your current marital status
10. Minors: If traveling with children, fill in their details here, including Name – Address – Nationality of the accompanying guardian.
For example: Accompanying person: Nguyen A, Hanoi, Vietnamese
11. National Identity Number: Identification number (Not mandatory)
12. Travel Document Type: Type of travel document
13. Travel Document Number: Passport number. The series is located on page 1 of the passport, below the national emblem. For instance: B0774312
14. Issue Date: Date of issuance, write exactly as in the passport. For example: 12/01/2017
15. Expiration Date: Date of expiration, write exactly as in the passport. For example: 12/01/2027
16. Issued By: Place of issuance. In the passport, this will be under Place of Issue. Fill in as per the passport, Immigration Management Department (Write in Vietnamese without accents)
17. Home and Email Address: Residential and email address
18. Residence in Another Country: Do you hold another citizenship? If yes, provide information about your second citizenship.
19. Current Occupation: Present occupation.
20. Employer or Educational Institution: Place of work, name, address, and phone number. If a student, you can fill in school information here.
21. The Main Purpose for Your Trip: Primary purpose of the journey. If it's tourism, mark the Tourism box.
22. Destination Member State: European country you will visit. Typically, if you're applying for a Schengen visa at the French consulate, you write France; or if applying at the Italian consulate, you write Italy.
23. First Entry Member State: The first country you will enter within the Schengen area. If your flight schedule lands in France first, you write France, similarly, if you land in Italy or Spain.
24. Number of Entries Requested: Number of times you plan to enter. Check the box for Multiple Entries, Single, or Two Entries.
25. Length of Stay: Number of days you will stay. Maximum for Schengen Type C, tourist visa is 90 days. In those 90 days, you can specify the number of days you intend to stay, for example: 30 days, 75 days, 90 days...
26. Schengen Visas Issued during the Last 3 Years: Number of Schengen visas obtained in the past 3 years.
27. Fingerprints: Have you provided fingerprints when applying for a Schengen visa? If this is your first time, mark No. Consulate will collect fingerprints when you submit your application. If you've provided fingerprints in previous applications, mark Yes.
28. Entry Permit for Your Final Destination Country, If Applicable: Information about the visa of the last country you are visiting (if applicable).
You can leave this section blank.
If you are not flying directly to Vietnam from any of the 28 Schengen countries, you may write the visa number of the last country you visited.
For instance, if your last country is Vietnam, or one of the ASEAN countries that Vietnam doesn't require a visa for, you can leave it blank.
If your last country is Egypt or Russia, fill in your visa number in this section.
29. Arrival Date: Estimated date of arrival in the Schengen area.
30. Departure Date: Date of leaving the Schengen area.
31. Inviting Person’s Name: Information of the inviting person. If you don't have an inviting person, you can write the name and details of the hotel you will stay at (Name + Phone number + Address).
32. Host Organization / Firm Name and Address: Provide the name and details of the hotel you will be visiting.
33. Travel and Living Expenses During Your Stay Are Covered by? Who will be covering your travel expenses?
34. Personal data…: Leave it blank unless you have someone in the three mentioned groups.
35. Family relationship…: Leave it blank unless you have someone in the three mentioned groups.
36. Date and Location: The time and place when you fill out the form. For example, Hanoi, July 6, 2017
37. Signature: Signature
