Step-by-Step Guide to Online Guardianship Registration
Online guardianship registration is only applicable to the Ward/Town level. To register online, follow these steps:
Step 1: Access registration HERE to register for guardianship.
Step 2: Under the 'Guardianship Registration' section, click on the 'District/Commune' box to select your district.
Then, the 'Receiving Facility' will appear, click on it to choose the appropriate receiving agency according to your needs.
Step 3: Afterward, fill in all the required information in the sections requested by the Public Service Portal. (*) indicates mandatory fields.
These include major sections such as:
- Requester Information
- Guardian Information
- Ward Information
- Reason for Guardianship Registration
Step 4: In the 'Other Information' section, click to request a copy of the online guardianship registration extract if you wish to obtain a copy, and enter the number of copies in the 'Number of Copies' box.
Step 5: Scroll down to the 'Attached Documents' section. In the bordered box, upload the attached documents file corresponding to each document title.
Step 6: Carefully review the filled information, then click on the 'I hereby take legal responsibility for the statements made above' box. Next, select 'Print Form' if needed, otherwise choose 'Continue' immediately.
