Guide to Online Termination of Guardianship
The agency at the District/Commune/Town level is the place to receive termination of guardianship documents; higher levels are not. Therefore, during registration, you must choose from the District/Town/Commune levels.
Step 1: Visit HERE. The interface will appear as shown below.
Step 2: Under the 'Register Termination of Guardianship' section, click on the 'District/Town' box to select the district or town where you reside. Then click on the 'Receiving Agency' box to choose the unit where you will submit your documents.
Step 3: Following that, in the 'Applicant Information' section, fill in the required and accurate information as requested by the Hanoi Public Service Portal. Remember to provide your personal phone number and email for easy contact. Visit the Mytour email registration post for a smoother email registration process.
Step 4: In the 'Guardian Information' section, fill in the required details similar to step 3.
Step 5: In the 'Ward Information' section, fill in the information similarly to the previous 2 steps.
Step 6: The 'Guardianship Registration Information' and 'Reason for Termination of Guardianship' sections follow the same process as above. This section can be left blank if necessary.
Note: Fields marked with (*) are mandatory and must be filled in with the required information.
Step 7: Go to the 'Attachment Profile' section; it is advisable to read carefully to upload the documents accurately and appropriately.
Step 8: After completing, double-check your entries. Then click on the 'I assume legal responsibility for the statements above' box. Press 'Print Form' if necessary, or choose 'Continue'.
Once you have entered all the required information, simply wait for the Hanoi Public Service Portal to send the confirmation of the termination of online guardianship to your email. That's it.
