Currently, getting vaccinated against Covid-19 is the safest approach during this pandemic. However, not everyone may have the means to register for vaccination. Therefore, let's explore how to register your loved ones for Covid-19 vaccine via phone and computer.
1. Registering Loved Ones for Covid-19 Vaccine via Phone
Step 1: To register your loved ones for vaccination, download the Electronic Health Book app. You can find the download link below:

Step 2: After installation, open the app and go to the Personal section at the bottom of the screen. Then, select the Family Members option to add your loved ones.
Step 3: Next, tap the green plus icon at the bottom of the screen and fill in the necessary information of your loved ones in the provided fields. Finally, click Save.
Step 4: Now you can start registering your loved ones. Return to the app's main screen and select the Vaccination Registration section. Then, under Name, tap to select your loved one again.
Step 5: If you've completed adding family members, simply click Continue. If not, fill in all fields, especially those marked with red asterisks. Next, in the Vaccination History section, declare any illnesses you may have and confirm by ticking the checkboxes.
Step 6: This is the final step. Read the instructions carefully and click Confirm in the Consent Form section to agree to the vaccination. On the final screen, your registration details will be displayed for you to review.
2. Registering Loved Ones for Covid-19 Vaccine via Computer
Step 1: Access the Covid-19 Vaccination Information Portal's website to begin registration. You can find the link below:

Step 2: After successfully accessing the website, hover over the Register button at the top right of the screen, then click on Individual Covid-19 Vaccine Registration to proceed with the declaration and completion process.
Step 3: Once you've entered the necessary information, especially the fields marked with red asterisks, proceed by clicking Continue to move on to the next declaration sections.
Step 4: In the Medical History section, carefully review whether you have any of the illnesses listed on the portal. If you do, mark the Yes checkbox; if not, select No, and if unsure, choose Not sure. After confirming your illnesses, click Continue.
Step 5: The information filling process is complete. Now, carefully read the important notes before registering for vaccination. If you decide to proceed, check the Vaccination Consent box and click Confirm.
Step 6: That's it! The final screen is for verifying the information you've registered. Please double-check to ensure everything is correct.
