Step 2: In the 'Registering as a Natural Guardian' section, click on the empty box labeled 'Receiving Authority' to choose where you want to submit your application.
Step 3: Fill in accurate and complete information in the fields required by Hanoi Public Services Gateway from 'Applicant Information', 'Guardian Information' to 'Ward Information'

- Ensure the information provided is accurate and complete
- It's best to fill out the information in sequence to avoid errors

Step 4: Scroll down to the section 'Additional Information', click on the box if requesting a copy and select the number of copies needed.
Step 5: Go to the section 'Attached Documents', within the table, look at the 'Attachment' box to upload the appropriate file corresponding to the required document name in the 'Document Name' column.
Step 6: Review to ensure the above steps are correct. Then click on the box 'I take legal responsibility for the statements above'. If you want to print the form, select 'Print Form' and then press 'Continue' or simply press 'Continue' directly.
