REPORT REGARDING PATIENT ESCAPE
At 6:30 AM, on November 12th, 2006, we, the undersigned, hereby document the escape of patient Quàng Văn Ún from the hospital:
- Attending physicians and nurses: Dr. Nguyen Nam, head physician, Dr. Le Dat, Nurse Tran Khanh.
- Patient in room 305, Lương Việt Thái, residing at Lò Văn Quảng.
Incident Summary:
- Patient Ún awaiting kidney stone surgery.
- Dr. Đạt discovered the patient missing at 9:00 PM on November 11th. Mr. Thái only knew Mr. Ún had left the room since 5:00 PM.
- At 10:00 PM, Mr. Ún still hadn't returned. Dr. Đạt and Nurse Khánh checked Mr. Ún's belongings and found them empty. Mr. Quảng remarked: Mr. Ún knows he needs surgery, he's very afraid.
Prediction: Mr. Ún, fearing surgery, has escaped from the hospital.
Request for hospital management to assist in locating Mr. Ún and persuade him to return to the hospital for necessary surgery.
