Here are the specific steps to follow when submitting a health declaration on the PC-COVID app:

Open Google Play or the App Store on your smartphone, and search for 'PC-Covid' in the search bar. The national PC-Covid app will appear as the first result. Tap on the 'Install' button and follow the usual installation steps.

- After successfully installing the PC-Covid app, open the app and enter your personal phone number. An OTP code will be sent to your phone. Enter the OTP code and tap the 'Activate' button. Once the OTP is successfully verified, tap 'Agree'. Your app is now activated.

Open the PC-Covid app that has been installed on your device, select 'Health Declaration' to complete the declaration before your flight.

Here, you can choose to declare domestic travel for yourself or for someone else.

The screen will navigate to the 'Travel Information' section. Here, you need to select your mode of transportation: airplane, train, bus, or ship. Since you're flying with Bamboo Airways, check the 'Airplane' option, then enter your departure date, departure location, destination, and more.

- In the past 14 days, have you traveled to any provinces or cities (possibly passing through multiple places)?

- In the past 14 days, have you experienced any of the following symptoms: cough, fever, difficulty breathing, body aches, loss or reduced sense of taste or smell?

- In the past 14 days, have you come into contact with: a person who is sick or suspected to have COVID-19, someone from a country with COVID-19 cases, or someone exhibiting symptoms such as fever, cough, difficulty breathing, or pneumonia?

After filling in all the required information, the passenger will press the 'Submit Declaration' button and receive a QR code associated with the declarant's name. This QR code will be scanned at the quarantine checkpoint.

- Passengers must provide truthful, complete, and accurate information as required.

- The information provided by passengers is guaranteed to be safe, secure, and used solely for disease prevention purposes, under the management of the National Steering Committee for COVID-19 Prevention and Control.

+ Proactively inform the local authorities where you reside or are staying.

+ Monitor your health and follow the specific COVID-19 prevention measures outlined by the local authorities upon your return.

+ If you experience any unusual health symptoms such as cough, fever, difficulty breathing, sore throat, or loss of taste, immediately notify the health authorities for monitoring and to implement the appropriate disease control procedures.
