Unexpected accidents happen to everyone. However, when it involves private areas, it’s not only painful but also quite embarrassing. Zipper accidents involving the genital area are common among children between 2-12 years old, but they can happen at any age. According to the British Journal of Urology International, more than 17,000 men and boys visited the emergency room for zipper-related incidents from 2002 to 2010. This accident occurs when the skin in the genital area gets caught in the zipper's teeth. Though this injury can be very painful, following these steps will ensure safe relief for your 'little one.'

Stay Calm. Pain and panic are the immediate reactions when injured, regardless of age. Before applying any methods, it is crucial to keep the injured person calm (even if it’s you).
- A child who is injured may become very frightened, so you need to check the injury while reassuring them.
- If the child is scared, calmly attract their attention to you or their favorite toy.

Apply mineral oil to the affected area. Mineral oil, containing kerosene, serves as a lubricant and helps reduce pressure on sensitive skin. Both mineral oil and petroleum jelly are known for their effectiveness in wound healing.
- Apply the oil to the genital area and zipper. This will help lubricate the area, allowing you to gently remove the skin from the zipper without having to cut it.
- Let the oil soak in for about 10-15 minutes.

Free the skin from the zipper. Try pulling the zipper head and skin in opposite directions. If it still doesn’t work, removing the zipper is the next step.
- One hand should hold the skin while the other grabs the zipper pull.
- Gently and carefully pull the skin and zipper apart in opposite directions.
- Note: If there is bleeding or fluid, stop immediately and seek medical assistance.

Remove the zipper. To release the skin, try removing the zipper from the bottom. This will help reduce the pressure between the zipper’s teeth and free the trapped skin.
- Method 1: Cut the connector at the front of the zipper pull (where the front and back of the zipper meet). This will help the zipper open, freeing the trapped skin with minimal pain.
- Method 2: Use the flat end of a screwdriver to insert between the zipper and teeth. Gently push the screwdriver back and forth towards the zipper pull connector to widen the gap and disrupt the zipper’s structure.
- Method 3: If methods 1 and 2 don’t work, use pliers to cut the teeth nearest the zipper head to free the skin.

Care for the injured skin. Once the zipper is removed, the skin will be sensitive and sore. However, it’s important to keep the area clean, stop any bleeding, and treat it properly.
- Wash the injured area with water and antibacterial soap.
- If bleeding occurs, apply a clean gauze directly to the wound until the bleeding stops.
- Maintain pressure on the wound for 15 minutes if bleeding is heavy. If the bleeding doesn’t stop, seek medical help.

To protect the wound from bacteria, apply Vaseline and place a sterile gauze on the injured skin. The Vaseline helps the gauze stick to the skin.

Observe signs of infection. Open wounds may lead to infections if not properly cared for. The alarming signs include:
- Red borders around the wound that spread
- Yellow or green discharge
- Red streaks rapidly spreading from the wound
- Swelling and pain
- Fever
- If any of these symptoms appear, seek immediate medical assistance
Treatment at the hospital

Go to the emergency room. If you cannot resolve the issue at home or feel uneasy, seek medical help.

Request pain relief or sedatives (if necessary). If the patient is extremely distressed, ask the doctor about using medications to help them calm down and proceed with the treatment.
- If the patient is calm enough, there is no need for sedatives, and local anesthesia can be used instead (see below).

Allow manipulation of sensitive areas. To manage the situation, the doctor will cut the garment, leaving only the zipper around the trapped skin.
- This helps to observe the area more effectively and reduces any unnecessary pressure the garment may cause on your skin.

Request local anesthesia. Local anesthesia (lidocaine injection) or topical anesthesia (EMLA cream) can be used to numb the area, which will help release the trapped skin.
- Surgical circumcision may be considered for particularly difficult cases or when a large area of skin is trapped.
- Note: Local anesthesia offers several advantages, but you must weigh those benefits against the discomfort that comes with injections in sensitive areas.

Request mineral oil application. This will help reduce friction and make it easier to release the trapped area. Let the oil absorb for about 15 minutes.

Follow the doctor's instructions to free the penis from the zipper. Various methods can be employed depending on where the skin is caught.
- If the skin is trapped between the teeth of the zipper, the doctor will cut above and below the area where the zipper meets the skin and gently pull apart the teeth.
- If the skin is caught in the slider section, the doctor may cut the slider's connector (the part where the front and back of the zipper meet) to destroy the mechanism and free the skin.

Consult with the doctor if an elliptical incision or circumcision surgery is necessary (only if absolutely required). If the skin remains trapped despite multiple attempts, an elliptical incision or circumcision should be performed by a urologist under general anesthesia.
