The Windows Insider Program offers tech enthusiasts early access to unfinished versions, or 'builds,' of Microsoft Windows 10 features. In exchange, Microsoft requests feedback from participants to improve the software before its official release.
However, this seemingly generous program backfired in 2017 when a Windows 10 update, intended only for insiders, ended up damaging the very product it was meant to enhance. The problematic build, labeled 16212, was never supposed to be released, even to insiders, as it was an internal project that accidentally leaked. Microsoft advised affected users to either wait for the next update or utilize the company’s Recovery Tool to resolve the issue.
Windows Mobile insiders faced a frustrating issue where they couldn’t update their smartphones. Attempting to do so triggered an endless reboot cycle, forcing their devices to restart repeatedly. The only solution was to disconnect from the Internet during the update process. Failing this, users had to rely on the Windows Recovery Tool, which wiped all personal data from their phones.

However, the Chattanooga waitress’s mistake occurred while he was off duty in 2017, leading her to accidentally drop a bag of cocaine into his water. Jekievea Monchell Yearby’s error resulted in her arrest for assault, possession of a controlled substance, and possession of drug paraphernalia. The officer alerted his colleagues, who reviewed the surveillance footage with the restaurant’s manager to confirm the incident.
Yearby was caught in the act and confessed that the bag of cocaine belonged to her, along with a second one hidden in her bra. She accidentally dropped the drug into the officer’s water glass while serving him. Although she claimed she doesn’t use or depend on drugs, her troubles didn’t end there. Not only was she arrested, but she also lost her job.
8. Stuntman Accidentally Strikes ‘Spider-Man’

The cookies weren’t just vaguely suggestive—they were unmistakably ithyphallic (the opposite of flaccid).

After Rhode Island lawmakers revised the prostitution law in 1980 to avoid infringing on First Amendment rights, streetwalking, pimping, and trafficking remained illegal. However, due to an oversight, prostitution itself became legal. Legislators accidentally removed the section of the statute that criminalized prostitution, leading to an unintended legalization of the practice.
This oversight meant prostitution was no longer a crime, as police discovered during Operation Rubdown, a sting operation targeting the vice. It took six years for lawmakers to rectify the mistake, making prostitution illegal once again in Rhode Island.
5. Kodak Stumbles Upon Evidence of Atomic Bomb Tests

In 1945, Kodak customers reported mysterious “black exposed spots” or “fogging” on their X-ray film. Julian Webb, a physicist in Kodak’s research team, theorized that the issue stemmed from accidental exposure to radium in the packaging materials. These materials, sourced from a wartime factory that also handled radium, likely contaminated the sensitive film, causing the unexpected fogging.
Webb found that the issue wasn’t caused by radium. Instead, the problem stemmed from a batch of strawboard, a rigid material used to separate film sheets. The strawboard contained an unfamiliar radioactive substance: Cerium-141, a byproduct of nuclear bomb explosions. In his 1949 report, Webb concluded, “The most plausible explanation for this radioactive contamination is that it originated from wind-carried fission products resulting from the atomic bomb detonation in New Mexico on July 16, 1945.”
Two years later, Kodak considered suing the US government for the “significant damage” caused to their products by the nuclear tests. Instead, they reached an agreement: the Atomic Energy Commission would inform Kodak in advance about atomic bomb test schedules, and in return, Kodak would keep the information confidential. Kodak had unintentionally uncovered the government’s atomic bomb tests, but the revelation remained classified.

In 2017, the Baltimore City Department of Housing declared a three-story row house unsafe following inspections. Police errors can have devastating consequences. In 2007, a heavily armed SWAT team in Minneapolis raided a two-story home, terrifying the family inside. Acting on incorrect information from an informant, the team believed they were targeting the hideout of a dangerous criminal. Instead, they had targeted the wrong house—a mistake that escalated quickly.
Around 1:00 AM, Vang Khang was asleep when his wife woke him. Believing his family was under attack by armed intruders, Vang grabbed his loaded shotgun and fired through the bedroom wall, hitting two officers outside. Thankfully, their protective gear saved them. Mistaking the situation for an ambush, the police returned fire, but no one was hurt. Vang’s 12-year-old son, realizing the men were police officers, informed his father, who didn’t speak English. Vang immediately ceased firing.
Vang was arrested but later released without charges. Police Chief Tim Dolan apologized for the officers’ error. Reflecting on the incident, Vang remarked, “Things could’ve ended very badly.” Fortunately, no tragedy occurred.
