What Are the Benefits of Under Eye Fillers?
Under eye fillers are a type of dermal filler, usually made from hyaluronic acid, injected into the hollow spaces beneath your eyes, such as tear troughs or orbital hollows. The objective is to replenish lost volume, restore balance, and reduce the visibility of dark circles or sunken eyes. They can also smooth out the area and diminish the appearance of fine lines.
“There are numerous dermal fillers available, such as Sculptra, Belotero, Radiesse, Restylane, and Juvederm,” explains Shawnda Dorantes, APRN, MSN, FNP-C, an aesthetic nurse practitioner at Beauty Lounge Medical Spa in San Diego, California. “The selection of the appropriate filler is made between the patient and provider during the initial consultation with an experienced injector.”
Is Under Eye Filler Safe?
Yes, under the right conditions. With an experienced injector and the appropriate candidate, under eye filler is generally considered safe and effective. There are several hyaluronic acid-based dermal fillers that are FDA-approved for use in under eye hollows, and the treatment is considered on-label, according to Kami Parsa, MD, a board-certified oculoplastic and reconstructive surgeon in Beverly Hills, California.
Robert Schwarcz, MD, a double-board certified oculoplastic surgeon practicing in New York City, concurs. However, he cautions, “Even though they are FDA-approved, it is crucial to exercise caution as this area is delicate and complex. Expertise in eye anatomy is essential.”

Who’s a Good Candidate for Under Eye Filler?
Under eye filler isn't suitable for everyone. It’s essential to consult with medically trained professionals to assess whether this treatment is the right option for your concerns, whether those are dark circles, wrinkles, or hollowness.
“The ideal candidates are those with genuine tear trough hollows who aren’t looking for more permanent, surgical options like fat transfer or lower eyelid blepharoplasty,” says Dr. Parsa. “The best candidates also don't have eye bags, don't retain fluid—medically known as malar edema or festoons—don’t experience fluctuating eye bags throughout the day, and don't have significant loose or crepey skin around the eyes.”
Any of these issues should be addressed through alternative treatments, as under eye fillers could potentially worsen them. Dr. Schwarcz points out that the best candidates are typically younger individuals with good skin quality and minimal existing fat. They generally see the best results from this treatment.
For those who aren't ideal candidates, Dr. Schwarcz suggests that more natural, long-lasting results may be achieved through procedures like blepharoplasty (eyelid lift), which can be combined with fat grafting and other treatments for a more natural, lasting outcome.
Under Eye Filler Cost
The cost of under eye filler can vary based on the individual. However, the average price range for under eye filler typically falls between $1,000 and $2,000 for both eyes. According to Dorantes, results usually last anywhere from six to 12 months, depending on the filler used and your personal metabolism.
What to Expect When Getting Under Eye Filler
Before any injections, you’ll have a consultation with a skilled injector to determine if you're a suitable candidate for under eye filler. If you're approved, your appointment will proceed with cleaning and prepping the area, ensuring it is free of makeup.
“Injections cause minimal discomfort, and most procedures take less than 15 minutes,” explains Dr. Schwarcz. “There’s little downtime after the procedure, and most patients return to their daily activities or work right after.”
You may experience some tenderness, but significant pain should not occur. To ease any discomfort, applying a cool compress right after, in the evening, and the following morning can help. Dr. Schwarcz advises avoiding intense exercise, alcohol, and blood-thinning medications like ibuprofen for the first 24 to 48 hours after the treatment.
