However, body acne, which can appear on the back, chest, buttocks, and other skin-covered areas, is often overlooked. We consulted dermatologists and skincare specialists to uncover the causes and effective treatments for body acne.
Understanding Body Acne
Body acne is a term used to describe acne that occurs on parts of the body other than the face. Fundamentally, acne is a widespread condition where hair follicles get blocked by a combination of dead skin cells and excess oil.
Regarding acne on the back and chest, Renee Rouleau, a renowned esthetician and skincare expert, notes that most individuals experience either large pustules or cysts. "Pustules resemble typical pimples, whereas cysts form deep beneath the skin, never surface, and can be painful. It’s crucial to avoid picking at cystic acne, as the body typically absorbs the infection internally."
Body acne frequently appears in consistent areas, such as the center of the chest, buttocks, and upper back. "These regions contain numerous oil glands, making them more prone to acne," says Ife Rodney, MD, a board-certified dermatologist at Eternal Dermatology. Additionally, you might encounter various forms of body acne, including fungal acne, which often develops after intense sweating.
Varieties of Body Acne
Body acne comes in different forms, and identifying the specific type you have is key to determining the appropriate treatment. Below are some of the most common varieties.
Acne Vulgaris
Acne vulgaris, the most prevalent form of acne, is identified by the presence of blackheads, whiteheads, papules, pustules, nodules, and sometimes cysts. This acne type arises when pores become blocked by excess oil, dead skin cells, dirt, debris, or bacteria.
Acne vulgaris often appears after physical activities like exercise. Sweat, a significant irritant, can aggravate the skin and block pores. "Exercise is a major trigger for body acne, particularly if you don't shower right after sweating," explains Dhaval G. Bhanusali, MD FAAD, a board-certified dermatologist based in New York City.
Acne Mechanica
Acne mechanica results from friction or heat and typically emerges on the shoulders, neck, and buttocks. It commonly affects athletes, individuals wearing tight clothing, and those using sports equipment. Carrying a heavy backpack can also lead to this acne type due to constant rubbing and friction from the straps. "Prolonged sitting can increase friction and occlusion in specific areas," notes Sara Perkins, MD, a board-certified dermatologist and member of the Hims & Hers Medical Advisory Board.
Acne Cosmetica
Certain beauty products, such as hair care items and makeup, can cause skin irritation. Acne cosmetica arises from cosmetics or skincare products that block pores. These products often contain comedogenic substances like oils, waxes, and silicones, which contribute to blackheads and whiteheads. This type of acne frequently appears along the hairline and on the neck. It is usually temporary, as it is directly linked to product usage.
Acne Conglobata
Acne conglobata is an extreme variant of acne vulgaris, marked by large nodules and cysts. It often results in significant scarring, particularly on the chest, back, and buttocks. This condition is typically triggered by a mix of factors, including hormonal imbalances (especially elevated androgen levels), genetic predisposition, and immune system irregularities.
Acne Fulminans
Acne fulminans is an uncommon and severe acne type that primarily affects teenage males. It manifests as sudden, intense inflammation accompanied by fever and joint pain.
Acne fulminans is frequently linked to elevated androgen levels (male hormones) and can worsen due to the use of anabolic steroids or specific medications, especially those that trigger hormonal shifts, sweating, dehydration, or other acne-inducing symptoms. For instance, corticosteroids can boost oil production and inflammation, leading to acne. Hormonal treatments, including certain contraceptives and testosterone replacement therapy, may also contribute to acne development.
Pityrosporum Folliculitis (Fungal Acne)
This acne type occurs in hair follicles due to an overgrowth of yeast. It appears as red bumps, commonly on the chest, back, and shoulders, and is often accompanied by intense itching. Unlike bacterial acne, fungal acne necessitates antifungal treatments for effective management.
Chloracne
Chloracne is a rare acne variant caused by exposure to chemicals such as chlorinated hydrocarbons. It manifests as cysts and pustules on the upper body and face. This condition is notoriously stubborn and challenging to treat.
Additional Causes of Body Acne
While certain types of body acne have clear triggers, such as friction or cosmetic products, other factors can also lead to breakouts. If the source of your acne isn’t obvious, consider these alternative potential causes.
Stress
Let’s address the most evident factor first: stress. Everyone experiences high stress at various times, and it’s natural for your skin to reflect that strain.
"Stress increases androgen production, leading to heightened inflammation and oilier skin," explains Rodney. "This excess oil blocks pores, causing acne. While stress doesn’t directly cause acne, it exacerbates existing conditions."
Menstrual Cycle
Hormonal acne is prevalent during menstruation. Research indicates that over half of menstruating individuals (63%) experience acne before their period.
Acne typically occurs during the follicular and luteal phases of the menstrual cycle. Hormonal contraceptives, such as birth control pills, can alleviate premenstrual symptoms (PMS) like acne, and certain PMS vitamins may also provide relief.
"Body acne often indicates a strong hormonal influence," says Rodney. "Hormonal acne commonly appears on the chest and back, areas where hair growth is prominent."
Alcohol
Alcohol can also contribute to body acne. This is because alcohol triggers inflammation and disrupts hormone levels, leading to increased oil production. Additionally, alcohol dehydrates the skin, prompting it to produce even more oil, which can clog pores.
Alcohol weakens the immune system, making it harder for the body to combat acne-causing bacteria. It also negatively impacts sleep, which is essential for the body to function effectively and fight off bacteria.
How to Treat Body Acne
As with any acne, addressing body breakouts requires a strategic approach. "Keep in mind that clearing acne takes time. You’ll need to address both internal and external factors contributing to your body acne," advises Rodney. Here are some tips for managing body acne.
Switch Soaps
Begin by switching to an acne-fighting body wash with ingredients like salicylic acid or benzoyl peroxide, suggests Perkins. "Benzoyl peroxide washes are ideal for body acne, but they should be left on for three to five minutes before rinsing for maximum effectiveness. Salicylic acid and glycolic acid are also excellent options, as they exfoliate dead skin cells and prevent pore blockages," she adds.
Avoid Irritants
Discontinue using products that irritate or harm your skin. Steer clear of oil-based shower gels, moisturizers, and haircare products, as these can clog pores. "If you’re prone to acne, watch out for panthenol," cautions Rouleau. "I discovered that my conditioner contained high amounts of it, and the residue on my back was a major acne trigger."
Use Body-Specific Products
While it might be tempting to use facial acne products on your body, Rodney advises against it: "Facial skin is typically more sensitive than body skin, so acne treatments designed for the face may not work as well on the body. Opt for body-specific products with stronger active ingredients for better results."
If you don’t already have one, consider using an antibacterial or anti-yeast formula, such as Head and Shoulders, as a body wash. Alternatively, opt for body washes specifically designed for acne that include salicylic acid as a key ingredient.
Maintain Good Hygiene
Proper hygiene is essential when addressing any form of acne. Showering right after exercise or sweating, along with wearing loose or moisture-wicking clothing, plays a crucial role in clearing body acne.
"For facial acne, I often advise patients to regularly change their pillowcases to prevent oil, dirt, and bacteria from re-contaminating the skin. The same principle applies to body acne," explains Rouleau.
Consider OTC or Prescription Treatments
Over-the-counter (OTC) products can be effective for treating mild body acne and breakouts. Popular options include products containing benzoyl peroxide, resorcinol, and salicylic acid.
For more severe or persistent body acne, prescription treatments may be necessary. A dermatologist or healthcare provider can recommend suitable products based on your skin type and condition. Common prescriptions include antibiotics, retinoids, and stronger formulations of benzoyl peroxide.
Preventing Body Acne Breakouts
While body acne is common, certain lifestyle and dietary changes can help prevent breakouts. Consider adopting these strategies to maintain clearer skin.
- Reduce stress. Stress increases oil production and inflammation, so managing stress through mindfulness or relaxation techniques can help prevent breakouts.
- Wash laundry frequently. Dead skin cells, sweat, and product residue can accumulate on fabrics. Regularly cleaning bedsheets, pillowcases, and clothes minimizes skin irritation.
- Apply sunscreen. Regular sunscreen use protects your skin and prevents excessive dryness, which can trigger oil overproduction. Choose a non-comedogenic sunscreen with at least SPF 30.
- Follow a balanced diet. Some studies suggest that diets high in oil, dairy, and sugar may worsen acne. Perkins highlights dairy as a key factor. Track your diet and adjust if you notice breakouts after consuming acne-triggering foods.
- Opt for loose clothing. Tight clothing restricts airflow, leading to breakouts. Breathable, sweat-wicking fabrics like Dri-fit are ideal.
- Stay hydrated. Proper hydration keeps your skin moisturized and reduces the likelihood of breakouts.
- Shower after sweating. Sweat can trap bacteria and toxins on the skin, so showering after sweating helps prevent acne.
- Exfoliate regularly. Gently exfoliate a few times a week to remove dead skin cells and prevent clogged pores. Use mild scrubs or products with salicylic or glycolic acid for best results.
For persistent or cystic hormonal body acne, consulting a dermatologist for a personalized treatment plan is advisable. They might suggest prescription-strength alpha hydroxy acid (AHA), beta-hydroxy acid (BHA), topical retinoids, hormonal treatments like birth control pills, or diuretics such as spironolactone.
