1. Lesson Plan 'Practicing Writing Report Texts' No. 4
A- KEY CONCEPTS
I- REVIEW THEORIES
1. What is the purpose of writing a report text?
2. What are the similarities and differences between report texts and reports?
3. What is the common structure of a report text? What essential elements should be included in this type of writing? How should the content be presented?
Answer:
1. The purpose of a report text is to present an event in an objective and accurate manner, so that the responsible party can fully understand the matter, leading to a correct assessment and conclusion.
2. Similarities: Both are types of documents sent from subordinates to superiors.
Differences: A report is usually periodic, while a report text is only created when an event occurs that requires an accurate, objective presentation for decision-making purposes.
3. The following elements are essential in a report text:
National title and slogan (centered).
Location and time of the report (top-right corner).
Document title (centered, in uppercase).
Sender (individual or organization).
Report content.
Request or pledge.
Signature and name of the writer.
B. EXERCISES AND SOLUTIONS
Exercise 1: page 137, 8th-grade textbook, Vol. 2
Identify the incorrect usage of report texts in the following situations:
a) A student frequently arrives late. The class teacher wants the student to acknowledge the issue and correct it. The student has written a report text to submit to the teacher.
b) To prepare for the Ho Chi Minh Youth Union Congress, the Youth Union leader wrote a report text.
c) The General Team Leader needs to know what the Youth Union has accomplished and its results during the recent campaign. Hoa, representing the Youth Union's leadership, writes a report text for the General Team Leader.
Solution: Situations (b) and (c) should have used a report instead of a report text.
Exercise 2: page 137, 8th-grade textbook, Vol. 2
Provide two common life situations where a report text should be written (excluding the situations mentioned in the textbook).
Example: A person loses their ID card and needs to report it to the police station. Nam fails to follow the rules and damages lab equipment, and writes a report text to submit to the teacher.
Exercise 3: page 137, 8th-grade textbook, Vol. 2
Based on a specific situation, write a report text.
Example:
SOCIALIST REPUBLIC OF VIETNAM
Independence - Freedom - Happiness
Nam Dinh, March 11, 2016
REPORT TEXT
On the damaged laboratory equipment
Dear: Mrs. Nguyen Phuong Anh, Chemistry teacher.
My name is Bui Anh Dung, an 8A1 student at Xuan Truong Secondary School, and I would like to report the following incident:
During the Chemistry class today, I accidentally broke 3 test tubes and 1 bottle of distilled water. I apologize for not following the laboratory rules. I take full responsibility for the mistake.
Reporter: Bui Anh Dung

2. "Practice Writing a Report" Exercise No. 5
I. THEORY REVIEW
1. The purpose of writing a report
Present the damages
Present the level of responsibility of the person reporting
The incidents that have occurred and require consideration of their consequences
2. The report and the document are similar in format and structure, including the time, location, and sequence of events. However, a report only focuses on the events with consequences and the identification of the reporter's responsibility.
A notification document only summarizes one issue, presenting both strengths and weaknesses, along with future plans.
3. The structure of the report consists of three parts. Review a) b) c) on pages 135 and 136.
II. EXERCISE
Question 1. Identify the errors in the usage of the report in the following situations:
a) This should be a REPORT in the form of a self-evaluation. Being late to school is a flaw that needs to be reported to the homeroom teacher for consideration.
b) Here, a summary and proposed action plan are required. Therefore, a REPORT is the most suitable.
c) What has been done and achieved should be written as a REPORT to discuss the successes, strengths, and areas for improvement.
Question 2. Two possible situations include:
(1) Mai is often mischievous and careless in class. During break time, Mai took Minh Nhật's literature book and hid it in Hạo Nhiên's bag. Later, Hạo Nhiên took the book out, thinking it was his. Upon being caught, Hạo Nhiên was wrongly accused of stealing the book. To clear up the misunderstanding, Hạo Nhiên writes a report to the homeroom teacher for clarification.
(2) During a physical education class, the teacher had previously instructed the class to meet at the swimming pool 3km away from the school. However, Hạo Nhiên had hurt his leg while mopping the floor and couldn't notify anyone. As a result, he didn’t go to the pool. The teacher marked him absent for disciplinary reasons, which led to a class suspension.
At the suggestion of the class and the homeroom teacher, Hạo Nhiên writes a report to ask the teacher for understanding and to lift the disciplinary action.
2. A possible version of situation (1) could be written as follows:
THE SOCIALIST REPUBLIC OF VIETNAM
Independence - Freedom - Happiness
REPORT
(Regarding missing physical education class)
Respected: Mr. Đặng Quang Thanh, Physical Education teacher for class 8A.
I am Thái Bảo Hạo Nhiên, a student in class 8A at Đặng Trần Côn Secondary School, and I would like to report the following incident:
On Monday (October 4, 2004), you instructed us to gather at the Hồ Hòa Bình swimming pool for swimming practice. Unfortunately, on Sunday (October 3, 2004), while helping my mother clean the house, I slipped and sprained my ankle. Due to this, I was unable to inform you in time, and though I attended other classes, I couldn’t make it to the swimming pool for the first period. I thought I would be able to meet someone at school to pass along a note, but due to heavy traffic, I couldn't find anyone.
I assure you that the above is true, and I kindly ask for your understanding to remove the disciplinary mark from the class register.

3. Lesson "Practice Writing Report Text" No. 6
Exercise 1. Identify the errors in the use of the text in the following situations:
a) A student is often late to school. The homeroom teacher wants the student to acknowledge the mistake and sincerely correct it. The student writes a report and submits it to the teacher.
b) To prepare for the Youth Union Congress, the team leader writes a report.
c) The team supervisor needs to know about the collective tasks completed by the team and the results from the recent competition, so Hoa, representing the team, writes a report for the supervisor.
Answer:
a) Incorrect; in this case, the student should write a self-critique report instead.
b) For the Youth Union Congress preparation, the team leader should write a report instead.
c) The supervisor needs to know about the team’s tasks and results from the competition, so Hoa, representing the team, should write a report, not a report text.
Exercise 2. List two situations that may require writing a report text (do not repeat examples from the textbook).
Answer:
Some situations that require writing a report:
– Students notice a missing flower pot and disorganized desks and chairs in the classroom.
– A student loses their school bag in the gymnasium.
– A group of 8A students block the road and bully a student from class 8C.
Exercise 3. Write a report based on a specific situation.
Answer:
THE SOCIALIST REPUBLIC OF VIETNAM
Independence – Freedom – Happiness
Hoa Binh, February 15, 2017
REPORT
Regarding the disorder that occurred in class on February 14, 2017
To: The School Board of Hai Tan Secondary School. My name is: Hoang Thu Hong, class leader of class 8H, Hai Tan Secondary School. On February 14, 2017, a disorder occurred in class 8H, and on behalf of the class, I would like to report the incident as follows:
At around 7:15 AM on February 14, 2017, during the 5-minute break between the first period (Chemistry) and the second period (History), when the teacher had just left the class, Tran Minh Viet, a student from class 9B, entered with a bouquet of flowers to give to Nguyen Thi Minh Hoa and asked Hoa to step outside. However, Hoa replied that it was a short break and leaving the class would violate school rules, so she refused. Viet agreed and suggested meeting during the break time. Viet then left the classroom, and Hoa placed the flowers on an empty desk at the back.
After the second period, at around 8:05 AM, when the teacher had left the room, and the class was packing their books, there was a loud banging on the classroom door. Tran Bich Van, a student from class 9B, along with a group of other female students, rushed in and headed straight for Hoa. Van started insulting Hoa, calling her derogatory names such as “stealing my boyfriend,” “you want to die,” etc. Nguyen Ngoc Truc then hit Hoa with the bouquet, and Nguyen Ngoc Minh slapped her. The students in class 8H were unable to react to the actions of the 9B students. Trần Hoa Bình quickly ran to call security. The male students in the class restrained the 9B group, which led to a brief scuffle. The situation stopped when the security guards arrived.
I affirm that the above account is true. If any of it is found to be incorrect, I accept any disciplinary actions from the school.
Writer of the report
Hoang Thu Hong

4. Exercise "Practice Writing a Report" No. 1
I. Theory Review
1. Purpose of writing a report: to present the damages or the extent of responsibility of the reporter in situations that need to be reviewed due to consequences.
2. Similarities and differences between reports and statements:
+ Similar: Both follow a structured format, including time, place, event, and relevant individuals.
+ Different: A report describes the actions taken and what has been achieved for others to know, while a statement describes the damage or responsibility involved in the event that needs further review.
3. A report must follow the correct format and provide accurate details including time, location, events, names of involved individuals, and the writer's suggestions.
A report should include the full names of the sender, recipient, and location.
II. Practice
Exercise 1 (page 137, Literature Book 8, Volume 2):
a) In this case, a statement of apology should be written.
b) Instead of a report, a plan for the event should be submitted.
c) Instead of a report, a progress report should be used.
Exercise 2 (page 137, Literature Book 8, Volume 2):
Some situations where a report is required:
- Students notice that a desk and a chair are missing in the classroom.

5. Lesson on "Practice Writing a Report Text" No. 2
Part I: THEORETICAL REVIEW
Question 1 (Textbook, page 136, Literature 8, volume 2)
What is the purpose of writing a report?
Answer:
The purpose of writing a report is:
- To present damages or losses.
- To assess the responsibility of the person reporting.
- To review the events that caused the damage, which need further consideration.
Question 2 (Textbook, page 136, Literature 8, volume 2)
What are the similarities and differences between a report text and a report document?
Answer:
Both the report text and the report document follow a similar format, providing details such as time, place, and event progress. However, the report text focuses solely on incidents with consequences and clarifies the level of responsibility of the reporter. A report document, on the other hand, presents a summary of a topic, with both its strengths and weaknesses, and outlines future plans or directions.
Question 3 (Textbook, page 136, Literature 8, volume 2)
What is the common structure of a report? What key elements must be included in this type of text? How should the content of the report be structured?
Answer:
A report must follow a formal format, clearly presenting the date, location, events, involved individuals, and the writer's recommendations. It should include the full names of the sender and receiver, and the place of submission.
Part II: PRACTICE EXERCISES
Question 1 (Textbook, page 137, Literature 8, volume 2)
Identify the mistakes in the following situations where a report is used incorrectly:
a) A student frequently arrives late to class. The homeroom teacher wants the student to acknowledge the mistake and correct it. The student submits a report to the teacher.
b) In preparation for the Young Pioneers' Congress, the head of the team writes a report.
c) The head of the group needs to know what the team has done and the results of their recent efforts. Hoa writes a report on behalf of the team for the teacher.
Answer:
a) The report should have been written as a self-assessment of the tardiness, not as a formal report. The teacher should review and consider the student's situation.
b) Here, the content should summarize and propose a plan, so a REPORT is more appropriate than a formal report.
c) In this case, a REPORT should focus on summarizing achievements and areas for improvement, rather than just stating what was done.
Question 2 (Textbook, page 137, Literature 8, volume 2)
Provide two real-life situations where you would need to write a report (avoid repeating the examples from the textbook).
Answer:
Possible scenarios:
(1) Mai is often mischievous in class. During recess, she secretly placed Minh Nhật's Literature 8 book in Hạo Nhiên's bag. When Hạo Nhiên mistakenly pulled out the book during class, everyone accused him of stealing. To clear his name, Hạo Nhiên writes a report to the teacher.
(2) During physical education, the teacher instructed students to gather at the swimming pool 3 kilometers away. However, Hạo Nhiên sprained his ankle while cleaning the house and couldn't inform anyone. He was marked absent and the class received a C-grade. To resolve the issue, Hạo Nhiên writes a report requesting understanding from the teacher.
Question 3 (Textbook, page 137, Literature 8, volume 2)
Write a report based on one of the situations above.
Answer:
The report based on scenario (2) would look like this:
THE SOCIALIST REPUBLIC OF VIETNAM

I had hoped to give the teacher an excuse letter, but I was delayed due to traffic.
I assure you that this incident is true and respectfully request your understanding to remove the C-grade from the attendance record.
Sincerely,
Thái Bảo Hạo Nhiên

REPORT
Regarding the loss of a wallet
To: Mrs. Class Supervisor, Class 8A3, Nguyen Tri Phuong Secondary School
My name is: Le Tran Hanh Mai, a student of Class 8A3, Nguyen Tri Phuong Secondary School.
On Tuesday, February 25, 2019, I lost my wallet in Class 8A3. Here are the details:
That day, I arrived at school at 7:15 AM. Before entering the classroom, my mother gave me 150,000 VND to hand over to the teacher for extra lessons. I carefully placed the money in a red wallet and put it into the middle compartment of my bag, which I securely closed before heading into the classroom. At 7:30 AM, we went outside for a review session and I left my bag and wallet behind. When the bell rang to return to class, I opened my bag to find the wallet was gone, and the bag was unlocked. I asked my classmates, but no one had seen or taken it.
The wallet contained 150,000 VND, a few family photos, a house key, and a 50,000 VND red envelope.
I swear the above statement is true. If any part is false, I accept any disciplinary action from the teacher and the class.
Signed,
Student
Hanh Mai
